The American Cancer Society (ACS), the Academy of Oncology Nurse and Patient Navigators (AONN+) and the Merck Foundation collaborated on a toolkit to facilitate patient navigation programs in the integration of 35 metrics identified and documented by AONN+ in 2015.
This toolkit summarizes how to use metrics effectively; includes pilot study that used 10 of the 35 metrics. Provides clear case studies and excellent background for how and why evaluation and tracking metrics are needed in patient navigation programs.
This toolkit will provide navigators with guidance on how to select, implement, report, and utilize navigation metrics as they engage in quality/ performance improvement and strategic decision- making.
The toolkit will illustrate how navigators can elevate their work by:
- Diving deeper into the use of metrics
- Investing in stakeholder relationships performance improvement leaders
- Engaging information technology and Creating dashboards to capture and manage navigation outcomes
- Using metrics to implement performance improvement initiatives for navigation
Audience: Administrators, patient navigation supervisory staff, patient navigators
